Patient Eligibility for Weight Loss Surgery

Choosing to undergo bariatric or weight loss surgery is a big decision. There are several important steps that must be completed to begin the process.

  2. Complete a registration packet, which you will receive after completing the seminar in person or online.
  3. Have first appointment with one of our surgeons.

    Our office will contact you after receiving your registration packet. At this point, itʼs important that you begin working with your health insurance provider to understand our practice requirements for surgery approval. At Synergy Bariatrics, these requirements include:

    • A completed primary care physician document form for bariatric surgery
    • A complete panel of laboratory tests
    • A pre-surgical psychological evaluation from a provider specializing in bariatric surgery (a list of providers in our area can be found in the registration packet)
    • Completion of a nutrition class (Synergy Bariatrics hosts a free session by Jennifer Turesky, MS, RDN, CDN, CDE)
    • Completion of a Fundamentals Workshops at Synergy Bariatrics

In addition, candidates for bariatric surgery must not be smoking cigarettes.

The criteria for surgery are based on your body mass index (BMI) and your history of attempts at sustaining weight loss through diet, exercise, or medicines. (Calculate your BMI »)

Option 1
  • Age 18 or older
  • Body mass index of > 40
  • A history of unsuccessful weight loss attempts through diet, exercise, or medication therapy
Option 2
  • Age 18 or older
  • Body mass index of > 35, with other chronic diseases such as high blood pressure, diabetes, and sleep apnea
  • A history of unsuccessful weight loss attempts through diet, exercise, or medication therapy

For successful and healthy weight loss, itʼs very important that patients understand the requirements and are motivated to work with our staff, follow up regularly after surgery, and adhere to the lifestyle changes we will teach you.

Most commonly, people that are motivated to continue with follow-up visits, lose the most weight, and actively participate in their treatment of other medical conditions have the greatest improvement in their quality of life.
