Erie County Medical Center Corporation’s Medical/Dental Staff is comprised of over 1,000 clinical providers working together to provide every patient with the highest quality of care delivered with compassion. ECMC’s Level 1 Adult Trauma Center, Comprehensive Psychiatric Emergency Program, Transplant Surgery, award-winning Long-term care in concert with a full complement of ambulatory care options, from primary care to outpatient specialty clinics offer clinicians a wide range of options in furthering their career or finding the right place to offer care to their patients.
Credentialing & Privileging
ECMCC is proud to partner with MD Staff in collaboration with Kaleida Health and Upper Alleghany Health System to provide a user-friendly, efficient credentialing platform. Through this portal you may request and complete an application for membership/privileges to any of the following facilities:
- Erie County Medical Center
- Kaleida Health System, including:
- Buffalo General Hospital/Gates Vascular Institute
- Millard Fillmore Suburban HospitalGolisano Children’s Hospital of BuffaloDeGraff Medical ParkHarlem Road Ambulatory Surgery Center

This new electronic platform in conjunction with our updated policies and procedures offer time-saving options for our valued providers such as one application submission for multiple facilities, both for initial and appointment and re-appointment.
It is important to note that while we have created as many shared efficiencies as possible, each organization is a separate and unique medical staff. You will be subject to the Bylaws, Rules and Regulations, policies and regulatory requirements of each, including application fees, medical staff dues, and orientation materials. Please read the instructions carefully, prior to submission.
Requesting an Application
You may request an application by emailing the following information to newappointments@ecmc.edu:
- Full legal name, including suffix (MD, DO, DDS, MBBS, etc.)
- NPI#
- Preferred email address
- Cell phone number
- Medical Specialty (Department to which you are applying)
- Which organization(s) are you applying to?
- Erie County Medical Center
- Kaleida Health System
- Upper Alleghany Health System
- (Optional) Name and email address for an authorized user to assist with the application. If opted, your personal review of the application and signature are still required.
- Attach a current copy of your CV.
The request will be reviewed by the appropriate Medical Staff Office. You will then be sent a link to MD-App and instructions to complete your application.

Application fees must be submitted to the organization(s) to which you are applying. Below is the information needed to do so. Please note that your application may not be processed until fees are received by the respective organizations.

Application/Processing Fees are non-refundable.
There will be a $250 late fee for reappointment applications received after due date.

Applying Online: Instructions
The following documents will be needed to complete your initial application for privileges. They can be uploaded to the MD-APP software when you are completing the application.
- Current copy of your New York State License Registration Certificate and a copy of your original New York State License (wall certificate)
- Copy of your current New York State Drivers License or other government issued photo identification (this must be presented to the Medical Staff office for verification)
- Information on citizenship/copy of current VISA if applicable
- Documentation of your malpractice insurance coverage (ECMC must be the Certificate Holder)
- Current copy of your DEA certificate
- Complete history & physical statement (include vaccination history and proof of PPD) Form will be sent to you with the application.
- Copy of your NYS infection control training certificate
- Current curriculum vitae (including a list of scientific papers or essays and national scientific meeting recently attended). Gaps of > 30 days must be explained per the application. CV MUST BE IN MO/YR FORMAT
- Any applicable diplomas from medical education and training (INCLUDING ECFMG IF APPLICABLE)
- Board certificates
- Other certifications (BLS, ACLS, NRP, PALS, APLS, etc.)
- Photo (JPG) to load into our credentialing system [optional; if none provided, we will use photo from ECMC name badge]
- All Allied Health Professionals must submit a signed, dated Collaborative or Supervisor Agreement.
Peer References
Three (3) references that may be contacted on your behalf should be indicated on the application, including an email address for each. This must be a professional that you have been affiliated with in the past 5 years and cannot be related by blood or marriage, or the ECMC Chief of Service in the Department to which you are applying.
Please utilize the link sent to you to access the MD-APP platform, complete your application and upload the appropriate documents. If you would like to delegate the form to a credentialing specialist within your practice, and have not done so already, you will have the ability once you access the application. PLEASE NOTE: You are personally responsible for the information contained the in the application and will need to review and sign the appropriate forms once it’s completed.
